Submissions are restricted to ten (10) single-sided pages maximum, and must include: (1) A positive statement of intent to bid as a prime contractor; (2) A completed and signed Sources Sought Response Form (attached below); (3) A listing of projects completed during the past three (3) years, which may include both Government and/or private industry projects, detailing the type of project, dollar value, contract number, location, and a point of contact for each, while also indicating if you were the prime or a subcontractor (if a subcontractor, provide the name and point of contact for the prime contractor); (4) A brief outline of resources, subcontractors, and key personnel that would be used to accomplish the contract; and (5) A letter from your surety detailing your maximum bonding capacity per project.
